USAC/USF Funding Process

Rural hospitals and public health departments are eligible to receive funding support of T-1 connections to the Network under the Universal Services Administrative Company.  Application to the USAC (or USF) must be made.  Please review these presentations for more information about applying for USF funding or visit the USAC website at www.universalservice.org

Nebraska Public Service Commission Funding Process

For not-for-profit hospitals that are eligible to receive Nebraska Public Service Commission funding, the forms below are available for your use.  Please read the document entitled Reimbursement Process before attempting to complete the necessary forms.  If you have questions, please contact your hub site. 

If you have already completed the necessary forms, you should receive a Support Schedule from the Nebraska Public Service Commission.  Please be sure to review the Support Schedule carefully for accuracy as this represents the funding that you are eligible to receive and that will be applied to your bill.  In addition, you should review your bill on a monthly basis to assure that all charges and credits are being applied accurately.
